The safety and welfare of your child is of the utmost importance to us.

We cannot stress enough why we need to have the most current information to contact you. Please notify the office if the following changes should occur:

1. Your home phone number has changed or is temporarily disconnected;

2. Your employer’s name and phone number has changed. Please include any extensions is necessary;

3. Your cell phone has been changed or is not in service;

4. Your address has changed;

5. Correct names and phone numbers of any one allowed to make decisions or pick up your child in the event of an illness or emergency.
