Physical exams are required for new enrollees and students in Kindergarten through 12th grade. Please see the Health Services page for the Universal Child Health Form for your physician to complete. You are encouraged to request that the completed form be sent to the school nurse at the completion of the physical exam. You may also bring a copy of the exam to your school nurse.
