Beginning on July 5th and running through August 4th Complete Care will be offering athletic physicals during morning hours (8am-12:30pm) at Vineland High School North. (Dennis Tanner Building)
Evening hours will also be available and will be offered at the Complete Care facility at 785 W. Sherman Ave.

Appointments for Sherman Ave. location can be made @ Completecarenj.org (online appointment scheduling).
Students must be accompanied by a parent and bring their insurance card, ID, and immunizations records to their appointment.
